Along with your application please include the following: 1. On a separate sheet, write a short essay 250 - 500 words -a personal statement about yourself, your interests, your experiences, and goals for the future. Also include why you want to be on the TKHN student committee and your ideas for a building a successful forum. If it's easier you may send a short audio or video tape. (Remember please do not send orginal tapes - we are not responsible for lost or damaged materials) 2. A written letter from a teacher or professor that will sponsor you Please note: if you are presently not attending school, you may have a child life specialist, playroom director or your doctor sponsor you Letter must be on the school, clinic or hospital stationary 3. Two reference letters - they can NOT be relatives At the top of the letter they should include their name, address, email & phone number Students if you need help with your application, please ask your parent/guardian, teacher or child life specialist to assist you. You can also email us to meet with one of us for an online chat. Students under 18 please have your parents/guardians look at our website and also review the material found here. If they have any questions tell them to contact us directly. We also welcome them to volunteer with TKHN |
